Autism Evaluation & Support Services

At EverBloom Holistic Nursing, we provide individualized autism evaluation services for children ages 3 to 13 years old. Evaluations are conducted by a board certified Psychiatric Mental Health Nurse Practitioner with experience working with children, adolescents, adults, behavioral health concerns, developmental differences, ADHD, anxiety, trauma related concerns, and family centered mental health care.

Our evaluation process is designed to help families better understand emotional, behavioral, sensory, communication, social, and developmental concerns while providing practical recommendations, guidance, and support planning in a compassionate and supportive environment.

The full evaluation process must be completed in order to obtain a detailed comprehensive clinical summary report and recommendations.

Appointment 1

Initial Clinical Consultation
60 to 75 Minutes

This appointment includes:
* Comprehensive developmental and behavioral history review
* Parent and caregiver interview
* Review of emotional, behavioral, social, communication, and sensory concerns
* Educational and school related history review
* Functional and adaptive behavior discussion
* Screening for concerns such as ADHD, anxiety, trauma, mood symptoms, and behavioral challenges
* Initial clinical impressions and evaluation planning

Families may also receive questionnaires, screening forms, and additional documents to complete before the next appointment.

Appointment 2

Structured Assessment and Behavioral Observation
75 to 90 Minutes

This session may include:
* Structured autism informed assessment activities
* Behavioral and social communication observations
* Emotional regulation and sensory processing review
* Functional and adaptive behavior assessment
* Review of completed questionnaires and collateral information
* Clinical interpretation and assessment findings

Assessment approaches are individualized based on age, developmental level, communication style, and clinical presentation.

Appointment 3

Feedback Session and Recommendations
45 to 60 Minutes

This appointment includes:
* Review of clinical findings and impressions
* Discussion of strengths and areas needing support
* Personalized recommendations and support planning
* Guidance regarding school accommodations, behavioral supports, therapies, social skills support, and community resources
* Opportunity for parents and caregivers to ask questions and discuss next steps

Following completion of all three appointments, families will receive a detailed comprehensive clinical summary report within 7 to 10 business days. The report may include observations, assessment findings, strengths, areas of concern, clinical impressions, and individualized recommendations for support, services, accommodations, and next steps when appropriate.

When indicated, families may also be referred to additional supportive services, therapies, behavioral programs, educational resources, social skills programs, parent support resources, or community based services to help support long term growth and success.

Comprehensive Autism Evaluation Package

$1,950

This package includes :
* All three appointments
* Parent and caregiver collaboration
* Review of records and completed questionnaires
* Clinical assessment and interpretation
* Feedback session and recommendations
* Detailed written comprehensive clinical summary report

These services are currently offered on a private pay basis only. Insurance is not currently accepted for autism evaluation services. Payment plans may be available upon request.
